About The Position

As a Home Health LVN/LPN, you’ll deliver skilled, one-on-one care in patients’ homes, using clinical judgment and working with autonomy to support patient outcomes and independence. You will monitor assigned cases to ensure care is delivered according to the Plan of Care, track patient progress, identify changes in condition, and report updates to the RN or physician. The role involves working in the field while staying connected to a supportive team.

The position is described as full-time. A full-time employee is regularly scheduled to work 30 or more hours per week or completes the productivity points required per week in the position agreement. Company-sponsored employee benefits begin on the first of the month following completion of the 90-day probationary period, subject to availability.

Requirements

• Current LVN license.
• Valid driver’s license.
• Valid auto insurance.
• CPR certification.

Responsibilities

• Partner with the RN to assess and implement individualized care plans.
• Provide skilled nursing care, including medication administration and wound care.
• Educate patients and caregivers on treatment and disease management.
• Communicate with the care team to support continuity and quality of care.
• Monitor assigned caseload for continuity, quality, and compliance.
• Track patient progress, identify changes in condition, and report updates to the RN or physician.
• Ensure care is delivered according to the Plan of Care.
