About The Position

The Home Health Field LVN/LPN delivers nursing care to patients in their homes, supporting comfort, safety, and effective management of health conditions. The role includes assessing patient health status, administering medications, providing wound care, and collaborating with physicians, therapists, family members, and other care team members. Under the direction of an RN, the LVN assists in identifying patient needs and participates in planning and implementing care in accordance with the plan of care. The position also involves educating patients and caregivers about treatment protocols and health maintenance, with the goal of supporting patient independence, recovery, chronic disease management, and improved well-being.

Requirements

• Current LVN license.
• Valid driver’s license.
• Valid auto insurance.
• CPR certification.

Responsibilities

• Under the direction of the RN, assist in identifying the patient’s physical, psychosocial, and environmental needs, as evidenced by documentation, the clinical record, team reports, and evaluations.
• Participate in planning and implementing care in conjunction with the RN and in accordance with the plan of care.
• Apply infection-control concepts and standard precautions when coordinating or performing patient care activities to prevent contamination and disease transmission.
• Assure continuity of care through the delivery of quality patient care.
• Communicate effectively with patients and families, team members, and other health care professionals, as evidenced by clinical notes, case conferences, communication notes, and evaluations.
• Monitor assigned cases to ensure compliance with third-party payor requirements.
• Promote the Agency’s philosophy and administrative policies to ensure quality of care.
• Assess patient health status, administer medications, provide wound care, and collaborate with physicians, therapists, and family members.
• Educate patients and caregivers on treatment protocols and health maintenance.
