About The Position

The Home Health Field LVN/LPN delivers nursing care to patients in their homes, supporting comfort, safety, health-condition management, recovery, and chronic disease management. The role includes assessing patient health status, administering medications, providing wound care, and collaborating with physicians, therapists, family members, and other care-team members. The LVN/LPN helps develop and implement individualized care plans that promote patient independence and well-being, and educates patients and caregivers on treatment protocols and health maintenance.

Under the direction of an RN, the Home Health LVN assists in identifying patient needs and participates in planning and implementing care according to the plan of care. The position applies infection-control and standard precautions, supports continuity and quality of care, communicates effectively with patients, families, team members, and other health professionals, monitors assigned cases for third-party payer compliance, and promotes the agency’s philosophy and administrative policies.

Requirements

• Current LVN license.
• Valid driver’s license.
• Valid auto insurance.
• CPR certification.

Responsibilities

• Under the direction of the RN, assist in identifying patients’ physical, psychosocial, and environmental needs, as reflected in documentation, clinical records, team reports, and evaluations.
• Participate in planning and implementing care with the RN in accordance with the plan of care (POC).
• Apply infection-control concepts and standard precautions when coordinating and performing patient-care activities to prevent contamination and disease transmission.
• Ensure continuity of care through delivery of quality patient care.
• Communicate effectively with patients and families, team members, and other healthcare professionals, as reflected in clinical notes, case conferences, communication notes, and evaluations.
• Monitor assigned cases to ensure compliance with third-party payer requirements.
• Promote the agency’s philosophy and administrative policies to ensure quality of care.
• Assess patient health status, administer medications, and provide wound care.
• Educate patients and caregivers on treatment protocols and health maintenance.
