About The Position
Accurate Home Care is seeking a Clinical Coordinator to support its Home Health team. The team helps patients remain at home and recover by managing their care, building relationships, and supporting a positive patient experience. The Clinical Coordinator supports the care team and helps organize patient care delivery. The position is based at the Otsego office and works Monday through Friday. Starting pay is $20.00 per hour; the salary range is undisclosed.
Requirements
- Working knowledge of Microsoft Office Suite.
- Excellent oral and written communication skills in English.
Nice To Haves
- A valid Minnesota Licensed Practical Nurse license is preferred but not required.
- Two years of home care experience is preferred.
Responsibilities
- Assist with coordination of the patient referral and intake process.
- Communicate with the nursing care team and physician offices.
- Organize care delivery, including scheduling, communication, and management of patient medical records.
