About The Position

Elara Caring is seeking a Licensed Practical Nurse Clinical Coordinator to support its home health teams and help deliver coordinated patient care. The role combines clinical coordination and administrative work with direct patient care through field visits as needed. The coordinator supports clinical workflows, maintains accurate documentation, and works with clinical staff and case managers to address patient needs.

Requirements

• Current, unrestricted Licensed Practical Nurse (LPN) license in the state of employment.
• Graduate of an accredited school of nursing.
• Minimum of 1 year of nursing experience in a clinical setting.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Ability to work in an office setting and travel within the assigned branch coverage area as needed.
• Valid driver's license and acceptable driving record, or reliable transportation to travel between assigned work locations.

• Bilingual in English and Spanish.

Responsibilities

• Support clinical teams in coordinating patient care and maintaining efficient workflows.
• Review and process Homecare Homebase (HCHB) coordination notes and administrative tasks accurately and on time.
• Obtain and enter physician orders for Clinical Team Manager review and approval.
• Monitor and maintain the At-Risk patient registry to support quality patient care.
• Assist patients with scheduling, authorizations, and questions or concerns.
• Complete patient chart audits and maintain accurate clinical documentation.
• Verify that required Medicare notices are provided to patients and maintain patient confidentiality.
• Collaborate with clinical staff and case managers to ensure coordinated patient care.
• Provide direct patient care through field visits as needed.
