About The Position

Elara Caring is seeking a Licensed Practical Nurse Clinical Coordinator who contributes to the delivery of home health care by supporting clinical teams, coordinating patient care, and maintaining efficient workflows. The role includes administrative and clinical coordination duties, patient support, documentation, and direct patient care through field visits as needed. The position involves working in an office setting and traveling within the assigned branch coverage area when needed.

Requirements

• Current, unrestricted Licensed Practical Nurse (LPN) license in the state of employment.
• Graduate of an accredited school of nursing.
• Minimum of 1 year of nursing experience in a clinical setting.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Ability to work in an office setting and travel within the assigned branch coverage area as needed.
• Must possess a valid driver's license, maintain an acceptable driving record, or have reliable transportation to travel between assigned work locations.

Responsibilities

• Support clinical teams in coordinating patient care and maintaining efficient workflows.
• Review and process Homecare Homebase (HCHB) coordination notes and administrative tasks accurately and on time.
• Obtain and enter physician orders for Clinical Team Manager review and approval.
• Monitor and maintain the At-Risk patient registry to support quality patient care.
• Assist patients with scheduling, authorizations, and addressing questions or concerns.
• Complete patient chart audits and maintain accurate clinical documentation.
• Verify required Medicare notices are provided to patients and maintain patient confidentiality.
• Collaborate with clinical staff and case managers to ensure coordinated patient care.
• Provide direct patient care through field visits as needed.
