About The Position

Residential Home Health and Hospice is hiring a Certified Nursing Assistant to provide home health care to patients in their homes in Glenview, Evanston, Highland Park, and surrounding areas. The role delivers compassionate, patient-centered care while supporting patients’ comfort, dignity, quality of life, safety, and independence. The position works independently in the field while collaborating with an interdisciplinary care team.

Requirements

• Active Certified Nursing Assistant (CNA) certificate in the state where the employee works.
• High school diploma or certificate.
• Minimum one year of healthcare experience.
• Valid driver’s license.
• Ability to spend approximately 20% of the workday driving to and from patient locations.

Responsibilities

• Provide hands-on care in patients’ homes, assisting with activities of daily living.
• Observe and report changes in patients’ conditions to support safety and independence.
• Reinforce care plans and healthy routines while supporting and educating patients and caregivers.
• Respond promptly to patient needs and communicate effectively with the care team.
• Maintain accurate and timely documentation in the EMR.
• Travel to patient homes within the assigned territory.
