About The Position

Residential Home Health and Hospice is hiring a Certified Nursing Assistant for a home health position serving patients in their homes in Glenview, Evanston, Highland Park, and surrounding areas. The role provides hands-on, patient-centered care that supports comfort, dignity, safety, independence, and quality of life. The CNA works independently while collaborating with an interdisciplinary care team and travels to patients’ homes within an assigned territory.

Requirements

• Active Certified Nursing Assistant (CNA) certificate in the state where the employee works.
• High school diploma or certificate.
• Minimum of one year of healthcare experience.
• Valid driver’s license.
• Ability to spend approximately 20% of the workday driving to and from patient locations.

Responsibilities

• Provide compassionate, patient-centered care in a hospice setting in collaboration with an interdisciplinary team.
• Assist patients at home with activities of daily living.
• Observe and report changes in patients’ conditions to support safety and independence.
• Educate and support patients and caregivers by reinforcing care plans, promoting healthy routines, and encouraging confidence with daily care tasks.
• Respond promptly to patient needs and communicate effectively with the care team.
• Maintain accurate, timely documentation in the EMR.
• Travel to patient homes within the assigned territory.
