About The Position

Join our team as a Home Health Business Development Liaison and play a pivotal role in connecting patients to high-quality care while helping grow a mission-driven organization. In this role, you’ll be the face of our home health services in the community—building trusted partnerships, expanding referral networks, and helping ensure patients receive the care they need, when and where they need it most.

Requirements

• Strong relationship-building and communication skills
• Knowledge of healthcare regulations and referral dynamics
• Valid driver’s license and auto insurance

Nice To Haves

• 2+ years of experience in home health or hospice sales
• Experience with CRM systems
• Familiarity with HomeCare HomeBase

Responsibilities

• Build and strengthen relationships with physicians, hospitals, case managers, and community partners to generate referrals
• Position Choice as the provider of choice through compelling presentations of our care model
• Identify new growth opportunities and support the development of innovative programs aligned with community needs
• Collaborate with clinical and operational leaders to ensure services meet market demand and regulatory requirements
• Partner with the interdisciplinary team to support safe, high-quality patient care
• Develop and execute strategic outreach and public relations initiatives
• Ensure full compliance with all local, state, and federal marketing regulations
• Consistently meet or exceed monthly admission and growth targets
