About The Position

At Brighton/Suncrest Hospice, the goal is to change expectations for hospice care by providing exceptional care and service to patients. The organization allocates resources to increase staff-to-patient ratios, increase clinical visits, and lower clinician caseloads. Brighton/Suncrest is a Community Health Accreditation Partner (CHAP)-certified hospice.

The Home Health Aide (HHA) works under the direction of a Case Manager/DON and provides care to assigned residents in accordance with resident assessments, care plans, and nursing management direction. The HHA works with licensed care staff to coordinate appropriate resident care and follows company nursing policies, procedures, and safety guidelines. The role involves one-to-one patient care, collaboration with a broader care team, and the ability to work independently.

Requirements

• Strong commitment to improving quality of life while partnering with community organizations in patient care.
• Ability to work well in a team environment and collaborate daily with the care team.
• Ability to work independently on a day-to-day basis.
• Willingness to drive, with reliable transportation, a valid driver's license, and auto insurance.
• Home Health Aide certification in the state(s) of practice.
• Certified Nursing Assistant certification in the state(s) of practice.

Responsibilities

• Provide care to assigned residents in accordance with resident assessments, care plans, and direction from nursing management.
• Work with licensed care staff to coordinate appropriate types of care for residents.

• Collaborate with nurses, social workers, chaplains, volunteers, and HHA leadership.
