About The Position

HomePointe HealthCare provides skilled, competent, and compassionate in-home care, primarily for children with acute or chronic illnesses and specialized medical needs. The Home Health Aide/Certified Nursing Assistant provides care in a child's home to support their health, potential, personal comfort, and activities of daily living. The role is offered as full-time or part-time, with schedules varying by location and school calendar. The position works under the direction of a Clinical Care Manager or Registered Nurse and collaborates with the interdisciplinary team.

Requirements

• Successfully complete a formal certification training program, written skills test, and competency evaluation.
• Complete agency evaluation as required by state and federal requirements.
• Have no continuous period of 24 consecutive months during which none of the certified services were furnished.
• Be currently listed in good standing on the state Certified Nurse Aide or Home Health Aide registry.
• Be at least 18 years of age.
• Be CPR certified.

Nice To Haves

• A minimum of six months’ work experience in a supervised setting, preferably in a health care facility.

Responsibilities

• Provide Home Health Aide services under the direction of the Clinical Care Manager or Registered Nurse.
• Perform services as necessary to maintain clients’ personal comfort and assist with activities of daily living.
• Perform household services.
• Report clinical information about the client’s status.
• May perform simple procedures as an extension of therapy or nursing services.
• Communicate with interdisciplinary team members through verbal reports and, as requested, participation in staff meetings and team conferences.
