About The Position

The Certified Nursing Assistant/Home Health Aide (CNA/HHA) works under the direction of the Nursing Leadership Team to provide compassionate care to assigned hospice patients in accordance with their assessments and individualized care plans. The CNA/HHA collaborates with the hospice care team to meet patient needs, promote comfort and dignity, and report changes in condition. The CNA/HHA follows all company policies, procedures, safety guidelines, and infection-control practices.

Requirements

• Be compassionate and committed to improving patients’ quality of life.
• Work collaboratively with nurses, social workers, chaplains, volunteers, and aide leadership.
• Be comfortable working one-on-one with patients while supported by a dedicated care team.
• Work independently and manage daily responsibilities.
• Be willing to travel and have reliable transportation, a valid driver’s license, and current auto insurance.
• Hold current CNA certification in the state(s) of practice.
• Hold current HHA certification in the state(s) of practice.
