We're currently seeking a Full-Time RN Case Manager to serve clients throughout Washington County, Minnesota, and St. Croix County, Wisconsin.

Requirements

  • Current, unrestricted Registered Nurse (RN) license in both Minnesota and Wisconsin
  • Current CPR certification
  • Negative TB skin test or chest X-ray
  • Minimum of one year of nursing experience in a healthcare.
  • Valid driver's license and auto insurance
  • Strong clinical assessment and critical thinking skills
  • Excellent written and verbal communication skills
  • Proficiency with electronic medical records and computer documentation

Nice To Haves

  • Home Care experience preferred

Responsibilities

  • Conduct comprehensive initial and ongoing client assessments
  • Develop, implement, and update individualized care plans in collaboration with physicians and the care team
  • Provide direct skilled nursing care to clients of varying ages and acuity levels
  • Coordinate care with clients, families, caregivers, and healthcare providers
  • Perform medication management, reconciliation, and client education
  • Monitor client conditions and communicate changes promptly
  • Educate and support clients, families, and caregivers
  • Complete timely and accurate clinical documentation using electronic charting
  • Assess home environments to promote safety
  • Provide clinical oversight and leadership to caregivers
  • Ensure compliance with OSHA, HIPAA, and all applicable state and federal regulations
  • Effectively manage your daily schedule and participate in an occasional on-call phone rotation
