We're currently seeking a Full-Time RN Case Manager to serve clients throughout Washington County, Minnesota, and St. Croix County, Wisconsin.
Requirements
- Current, unrestricted Registered Nurse (RN) license in both Minnesota and Wisconsin
- Current CPR certification
- Negative TB skin test or chest X-ray
- Minimum of one year of nursing experience in a healthcare.
- Valid driver's license and auto insurance
- Strong clinical assessment and critical thinking skills
- Excellent written and verbal communication skills
- Proficiency with electronic medical records and computer documentation

- Home Care experience preferred
Responsibilities
- Conduct comprehensive initial and ongoing client assessments
- Develop, implement, and update individualized care plans in collaboration with physicians and the care team
- Provide direct skilled nursing care to clients of varying ages and acuity levels
- Coordinate care with clients, families, caregivers, and healthcare providers
- Perform medication management, reconciliation, and client education
- Monitor client conditions and communicate changes promptly
- Educate and support clients, families, and caregivers
- Complete timely and accurate clinical documentation using electronic charting
- Assess home environments to promote safety
- Provide clinical oversight and leadership to caregivers
- Ensure compliance with OSHA, HIPAA, and all applicable state and federal regulations
- Effectively manage your daily schedule and participate in an occasional on-call phone rotation
