About The Position

Healthy At Home Caregivers is seeking an experienced Outreach & Community Relations Manager to build referral relationships and help more people access dependable care at home. Based in Orange County, California, this role focuses on developing relationships that generate new client referrals and growth in billable care hours. The manager will work directly with the owner and office staff and contribute to the agency’s growth by building lasting community relationships. The role involves identifying and developing referral sources, maintaining existing relationships, representing the agency in the community, and managing outreach activity and results. The manager will coordinate with care coordination and scheduling staff to support transitions from referral to service and work with the digital marketing provider to align online materials with outreach activities. Success is measured by growth in weekly billable care hours and new referral sources that generate client referrals.

Requirements

At least three years of business development, outreach, or community relations experience in home care, home health, hospice, or senior services.
Understanding of how healthcare and home care referrals are generated.
Ability to build trust and maintain productive professional relationships.
Confidence speaking with healthcare professionals, families, and community groups.
Strong organization and follow-through, with the ability to manage an independent schedule and priorities.
Willingness to take ownership of goals and report progress consistently.
Commitment to helping people remain safely and comfortably in their homes.

Nice To Haves

Established relationships within the Orange County healthcare and senior services community.
Familiarity with CalOptima, managed care organizations, VA programs, or other community-based services.
Experience using a CRM or similar system to manage outreach and referrals.
Experience delivering professional presentations and community education.

Responsibilities

Develop referral relationships with hospitals, discharge planners, case managers, physicians, skilled nursing facilities, hospices, senior living communities, and community organizations.
Identify and develop new referral sources while strengthening relationships with existing sources.
Meet regularly with referral contacts to understand their needs and explain how Healthy At Home can help.
Represent Healthy At Home at networking events, educational programs, and community activities.
Manage an organized outreach pipeline, documenting contacts, follow-up actions, referrals, and results.
Work with care coordination and scheduling staff to support a smooth transition from referral to service.
Identify opportunities within managed care and community-based programs.
Coordinate with the digital marketing provider so online materials support outreach activities.
Report progress consistently and take ownership of goals.
