RN Home Care Case Manager
About The Position
BrightStar Care is seeking a Registered Nurse to serve as a Home Care Case Manager four or five days per week in the Burnsville area and surrounding communities, including Eagan, Inver Grove Heights, and Apple Valley. The salaried role works closely with a team of RNs and LPNs to complete assessments, reassessments, and selected skilled nursing tasks for private-pay and veteran clients. As a non-Medicare agency, BrightStar Care does not require OASIS documentation. The nursing team is described as collaborative and supportive, with a focus on helping clients remain safe and comfortable in their homes. Four days per week pays $65,000 annually; five days per week pays $82,000 annually. The position offers paid mileage between client visits and the office, PTO accrual starting on the first day, and flexible scheduling.
Requirements
- Active RN license in the state of Minnesota.

Responsibilities
- Assess, plan, implement, and evaluate nursing care in accordance with professional standards.
- Collaborate with multidisciplinary team members to optimize patient care and outcomes.
- Educate patients and their families on health maintenance and disease prevention.
- Ensure accurate documentation and compliance with regulatory guidelines.
- Complete assessments, reassessments, and selected skilled nursing tasks for private-pay and veteran clients.
