About The Position

Care Synergy has an immediate opening for a Health Information Management (HIM) Coordinator. This role provides support functions to maintain an effective and efficient Health Information Management (HIM) department that is compliant with all state, federal, accreditation and internal policies related to agency documentation and records. The HIM Coordinator ensures confidential clinical records meet regulatory standards for accuracy and completeness of hospice documentation. They complete the electronic documentation, paperwork and computer data entry for all new admissions, and assure that all forms have required signatures and medical coding is correct. This role provides support to clinical staff for all health information requests, maintains tracking logs, and completes quality review for each of census, patient transfers, recertification orders, benefit certifications, etc. The HIM Coordinator also completes a quality audit of all charts at the time of patient death/discharge with communication to appropriate parties, tracks physician licensure and disciplinary actions, and provides documentation to external agencies for collaboration of patient care. They coordinate with external agencies for release of protected healthcare information (PHI) according to HIPAA standards, maintain clinical records after patient discharge/death per regulatory requirements, and participate in the organization’s quality assessment and performance improvement (QAPI) program.

Requirements

  • Minimum Education: High school diploma.
  • Minimum Experience: One year of work experience in a health care setting.
  • Required License: Valid Colorado driver’s license and proof of automobile insurance in accordance with state statute.
  • Ability to lift/carry a minimum of 30 lbs
  • Requires approximately 10% travel to Affiliate locations throughout the Front Range

Nice To Haves

  • Postsecondary training/education preferred.
  • EMR experience preferred.

Responsibilities

  • Completes the electronic documentation, paperwork and computer data entry for all new admissions.
  • Assures that all forms have required signatures and medical coding is correct (if applicable, per affiliate).
  • Provides support to clinical staff for all health information requests.
  • Maintains tracking logs and completes quality review for each of census, patient transfers, recertification orders, benefit certifications, etc.
  • Completes quality audit of all charts at time of patient death/discharge with communication to appropriate parties (coroner, attending physician, tumor registries, etc.)
  • Tracks physician licensure and disciplinary actions as requested.
  • Provides documentation to external agencies for collaboration of patient care.
  • Coordinates with external agencies for release of protected healthcare information (PHI) according to HIPAA standards.
  • Maintains clinical record after patient discharge/death per regulatory requirements.
  • Participates in the organization’s quality assessment and performance improvement (QAPI) program at a departmental or agency-wide level as requested.
