About The Position

The Health Care Director (HCD) leads health and wellness activities for the community’s residents and manages day-to-day healthcare operations. The HCD evaluates prospective residents before move-in to determine whether their care and service needs can be met, conducts reevaluations as needed, and develops and revises individualized service plans based on each resident’s evaluation, preferences, condition, and applicable regulations. The HCD participates in interdisciplinary reviews of service plans with residents, family members, physicians, healthcare providers, community leaders, and wellness team members. The role also collaborates with residents’ physicians, pharmacists, and other healthcare professionals to coordinate care. The HCD is responsible for the community’s medication management program, including collaboration with the pharmacy, audits, physician orders, controlled-drug oversight, and other activities that support safe medication use. The HCD supervises medication technicians, Approved Medication Assistive Personnel (AMAP), and other approved and trained unlicensed personnel who administer medications. The HCD provides training and validates skills or observes medication passes as required by State regulations. The role also supervises and trains CNAs and other direct care personnel, delegates within the scope of the HCD’s nursing license, and ensures documentation and the resident care department are survey ready. The HCD participates in developing and following plans of correction, serves as the community’s infection preventionist, and oversees efforts to prevent or limit communicable illness. The HCD monitors resident-care and quality indicators, raises trends in weekly At-Risk or monthly QAPI meetings, and leads hiring, onboarding, training, performance evaluation, coaching, and team development.
