About The Position

As a Health Home Care Coordinator - Spanish English, you will support members with chronic conditions by delivering patient-centered care coordination and reducing gaps across medical, behavioral health, and Social Services. This onsite, full-time role in Tacoma, WA ensures clients receive the right care at the right time through a structured Health Action Plan (HAP) and ongoing Case Management. Key responsibilities include care planning; interdisciplinary collaboration across care settings; care transitions using the Coleman Model/Care Transition Intervention (CTI); evidence-based screenings (BMI Assessment, PHQ-9, GAD-7, Katz ADL); Patient Activation Measure (PAM); referrals to community resources and Long-Term Services and Supports (LTSS); client advocacy; and accurate, timely documentation aligned with healthcare regulations and client confidentiality/HIPAA compliance.

Requirements

  • Bilingual Spanish and English (Spanish Speaking)
  • AAS degree
  • 1+ year Behavioral Health Care Coordinator, case manager, or social service coordinator experience
  • WA State Driver’s License
  • Auto insurance
  • Ability to pass DSHS/ALTSA background check

  • Completing required documentation by monthly deadlines
  • Coordinating follow-ups after transitions in care
  • Responding to client questions with clear, culturally responsive communication
  • Organizing case notes
  • Scheduling next steps
  • Readiness for team check-ins or training
