About The Position

Health Home Care Coordinator - Vietnamese English KWA Home Care is a non-profit multicultural social service agency. As a Health Home Care Coordinator - Vietnamese English, you will support members with chronic conditions by delivering patient-centered care coordination and reducing gaps across medical, behavioral health, and Social Services. This onsite, full-time role in Tacoma, WA ensures clients receive the right care at the right time through a structured Health Action Plan (HAP) and ongoing Case Management.

Requirements

  • Bilingual Vietnamese and English (Vietnamese Speaking)
  • AAS degree
  • 1+ year Behavioral Health Care Coordinator, case manager, or social service coordinator experience
  • WA State Driver’s License
  • Auto insurance
  • Ability to pass DSHS/ALTSA background check

Responsibilities

  • Deliver patient-centered care coordination.
  • Reduce gaps across medical, behavioral health, and Social Services.
  • Develop and manage a structured Health Action Plan (HAP).
  • Provide ongoing Case Management.
  • Conduct care planning.
  • Collaborate with interdisciplinary teams across care settings.
  • Facilitate care transitions using the Coleman Model/Care Transition Intervention (CTI).
  • Perform evidence-based screenings (BMI Assessment, PHQ-9, GAD-7, Katz ADL).
  • Administer the Patient Activation Measure (PAM).
  • Make referrals to community resources and Long-Term Services and Supports (LTSS).
  • Advocate for clients.
  • Complete accurate and timely documentation aligned with healthcare regulations and client confidentiality/HIPAA compliance.
  • Review caseload, prior notes, and upcoming touchpoints.
  • Prepare for monthly in-person visits with Vietnamese Speaking members.
  • Conduct interviews to confirm goals, barriers, and progress.
  • Update the Health Action Plan (HAP) accordingly.
  • Connect with clinics, behavioral health partners, and community agencies to keep services aligned and timely.
  • Coordinate follow-ups after transitions in care.
  • Respond to client questions with clear, culturally responsive communication.
  • Organize case notes and schedule next steps.
  • Participate in team check-ins or training.
