About The Position

This is a professional Health Education Consultant position in the field of Public Health working with the Family and Personal Health Healthy Start Program at the Collier County Health Department, planning, implementing and evaluating health promotion and Public Health Education programs for at risk pregnant women and infants. This position requires the individual to be self-motivated and work independently away from direct supervision. Contributes to the organization’s culture of performance excellence by actively seeking to improve personal and organizational performance management committees, councils, and/or workgroups. Continuously demonstrates the Departments ICARE values. This position also provides field service support to other Health Department Divisions as requested. This position reports to the Health Services Manager A-SES. Implements the Healthy Start Program to families referred to and enrolled in Healthy Start in a manner consistent with the Healthy Start standards and guidelines and the Healthy Start Coalition contract. Responsible for outreach, initial intakes (IIs), Initial Assessments (IAs), and ongoing care coordination services for families. Assists families in assuring access to prenatal or infant care and community resources. Assists clients with forms. Provides the Healthy Start services of care coordination, smoking cessation, outreach, and parenting, childbirth, and breastfeeding support/education. Provides services in a location that best meet the needs of the family (i.e. home visits, clinic visits, and alternative settings). Provides Healthy Start level specific and risk appropriate services. Monitors Healthy Start participants’ access to prenatal and infant services. Responsible for the development of Family Support Plans. Participates in service delivery planning. Meets or exceeds performance standards. Process and enters Collier County Healthy Start Prenatal and Infant Risk Screening form data into the Health Management System (HMS) and Well Family System (WFS). Works in conjunction with the Collier County Program Manager to systematically file/store screening forms. Acting backup in processing and entering screening data in absence of the Program Manager. Performs face-to-face initial Intakes (IIs) as needed/opportune. Documents care coordination services and maintains/updates client files via WFS per Healthy Start standards and guidelines. Performs qualify improvement functions. Acts as a liaison with community agencies and service organizations through referral and transitional services. Attends scheduled meetings, training sessions, and outreach events. Promotes public awareness of the Healthy Start Program and develops and maintains community relationships to support client referrals. Assists with program reports and diaper log. Covers program administrative needs in the absence of the Health Services Manager. Enters EARS and services into HMS according to agency protocols and timeframes. Attends staff meetings and completes required in-services and trainings. Performs other duties as assigned.

Requirements

  • Knowledge of Maternal/child health.
  • Skilled in-Home visiting experience, preferably Healthy Start.
  • Skilled with Health Management System (HMS)
  • Ability to deliver health education services.
  • Ability to plan, organize and coordinate work assignments independently.
  • Ability to communicate effectively, clearly, and concisely through oral and written means.

  • Minimum of four-year college degree in one of the following areas: Social sciences, A health-related field such as nursing, health education, health planning, or health care administration, Social work.
  • Two years of college with three years of public health/maternal-child health experience.
  • This position is at risk of exposure to blood and airborne pathogens.
  • This position is required to follow security guidelines established by the agency.
  • Valid Driver’s License and reliable transportation.
  • Incumbent may be required to work before, during and/or beyond normal work hours or days in the event of an emergency. Emergency duty required of the incumbent includes working in special needs or Red Cross shelters, or performing other emergency duties including, but not limited to, responses to or threats involving any disaster or threat of disaster, man-made or natural.
  • All Florida Department of Health positions require the incumbent to be able to learn and communicate effectively, orally and in writing, in English.

Nice To Haves

  • Experience working in a public health or health education role, specifically in maternal child health programs.
