About The Position

Under the leadership and direction of the Public Health Officer, the Health Educator position is responsible for assisting with health education services for American Indian individuals receiving services from the Tule River Indian Health Center, Inc. (TRIHCI). Health education may take place at the primary location and satellite office, associated clinics and agencies, or outreach sites, including client's homes. The Health Educator is responsible for leading groups or co-leading groups on various educational topics. The Health Educator will provide individual education to TRIHCI clients on various topics, including nutrition, diabetic coaching, and motivational interviewing. The Health Educator is part of a multi-disciplinary team working to establish services that enhance the health and well-being of the clients while developing their self-monitoring and/or healthier behaviors. Health Educator uses research skills to define the educational needs of the target population; design, manage, and evaluate health education programs; and serve in a primary role in the provision of services to TRIHCI patients and other residents of the service area. The position requires specialized skills and the ability to work well as a member of a multidisciplinary team.

Requirements

  • Good communication skills, especially the ability to effectively communicate with diverse populations – youth, tribal leaders, health and social services professionals.
  • Strong organizational skills and attention to detail to track all grant deliverables.
  • Demonstrate the ability to work independently and reliably, particularly in coordinating events such as community meetings, trainings, and health education seminars.
  • Basic knowledge of office and healthcare technology, such as Office 365 package, CANVA, communications and listserv software, social media tools and electronic health record systems.
  • Ability to maintain accurate records, prepare reports, and ensure all health information is handled confidentially and properly documented.
  • Ability to work with families and understand the cultural sensitivities in a Native American community.
  • Must maintain a professional appearance and attitude while engaging with patients and their families.
  • Must have a Valid California Driver License and be insurable with the Tule River Indian Health Center Inc. insurance agency.
  • First Aid/BLS/CPR certification required or must be obtained within 3 months of hire.

Nice To Haves

  • Academic or professional experience with grants management, administration, and data collection is highly preferred.
  • Familiarity with public health programs frameworks such as policy, systems and environmental (PSE change) and policy development is preferred.
  • Community healthcare experience is preferred, but not required

Responsibilities

  • Identify individual and community health education needs and implement a strategy to address those needs one-on-one or in group education events.
  • Complete training programs as needed to gain subject matter expertise in key areas.
  • Responsible for conducting individual one-to-one health education sessions on diabetes, nutrition, self-care, or other health topics and developing a follow-up plan to monitor behavior modification.
  • Deliver classroom presentations regarding general health (i.e., sleep, rest, safety, CPR. chronic illness, Diabetes, Hyperlipidemia, Hypertension, etc.) as well as specific health topics (i.e., drugs, alcohol, teenage pregnancy, pesticide awareness).
  • Work closely with case management and departments to ensure education for diabetes clients according to key indicators and Indian Health Service diabetes audit indicators to ensure each client receives the standard screenings, evaluations, and follow-up.
  • Develop health education materials in collaboration with other staff to share publicly with the TRIHCI patient population.
  • Lead delivery of Health Promotion/Disease Prevention activities.
  • Support the program by submitting monthly activity reports to the Public Health Officer for grant requirements and deliverables.
  • Participate in all required webinars, trainings, and workshops as determined necessary or required for related grant programs.
  • Participate in the Quality Improvement committee meetings and works with other department supervisors to ensure health promotion and prevention focus is included in all center programs and services.
  • Coordinate with in-house service providers to develop, implement, and monitor an individual care plan for each patient.
  • Assists in the research of proposals for grant applications or renewals.
