The Health Coordinator supports Pioneer Human Services’ mission by coordinating and monitoring clients’ physical and behavioral health services in residential treatment facilities. The role helps ensure continuity of care by facilitating communication among clients, nursing staff when present, medical and behavioral health providers, pharmacy services, and program staff. Duties include supporting intake, coordinating medical and behavioral health appointments—including telehealth—assisting with medication monitoring and electronic medication administration record (eMAR/MAR) documentation, and gathering and communicating relevant health information to clinical staff. At sites with licensed nurses, the Health Coordinator works under their direction to support care coordination and implementation of provider orders. At sites without nurses, the role works under the direction of the Health & Resource Coordination Unit (HRCU) Manager. The Health Coordinator also provides direct support in a structured residential setting. This includes engaging clients in the milieu, observing and documenting changes in physical or behavioral health, reinforcing treatment plans and health education, helping clients access healthcare, and responding to urgent health concerns by notifying clinical staff or emergency services as appropriate. Responsibilities involving medication handling and clinical coordination may vary by site or unit based on staffing, program licensure, and the presence of licensed nurses. This is a full-time swing-shift position. New hires start at $20.77–$23.35, with individual compensation determined by experience, qualifications, skills, competencies, internal equity, and market factors. The employer describes its benefits as a Total Rewards package supporting employees’ health, security, and balance.
