Health Coordinator FT/NOC

Pioneer Human Services•Sedro-Woolley, WA
•$17 - $28

About The Position

The Health Coordinator supports Pioneer Human Services’ mission by coordinating and monitoring clients’ physical and behavioral health services within residential treatment facilities. The role helps maintain continuity of care by facilitating communication among clients, nursing staff where present, medical and behavioral health providers, pharmacy services, and program staff. Responsibilities include supporting intake; coordinating internal and external appointments, including telehealth; assisting with medication monitoring and accurate electronic medication administration record (eMAR/MAR) documentation; and gathering and communicating health information to clinical staff. At sites with licensed nurses, the Health Coordinator works under nursing staff’s direction to support care coordination and provider orders. At sites without nurses, the role works under the direction of the Health & Resource Coordination (HRCU) Manager. The position also provides direct support in a structured residential setting: engaging clients, observing and documenting changes in physical and behavioral health, reinforcing treatment plans and health education, helping clients access healthcare, and responding to urgent health concerns by notifying clinical staff or emergency services. The role requires strong organization and attention to detail when supporting clients with complex medical, behavioral health, and substance use needs. Specific medication-handling and clinical-coordination duties may vary by site or unit.
