The Health Coordinator supports Pioneer Human Services’ mission by coordinating and monitoring clients’ physical and behavioral health services in residential treatment facilities. The role helps maintain continuity of care by facilitating communication among clients, nursing staff where present, healthcare providers, pharmacies, and program staff. Duties include supporting intake, coordinating medical and behavioral health appointments—including telehealth—assisting with medication monitoring and accurate electronic medication administration record documentation, and gathering and communicating relevant health information to clinical staff. At sites with licensed nurses, the Health Coordinator works under nursing staff’s direction to support care coordination and implementation of provider orders. At sites without nurses, the role works under the direction of the Health & Resource Coordination Unit Manager. The coordinator also provides direct support in a structured residential setting by engaging clients, monitoring changes in health status, reinforcing treatment plans and health education, helping clients access healthcare, and responding to urgent health concerns by notifying clinical staff or emergency services as needed. Responsibilities involving medication handling and clinical coordination may vary by site or unit. Pioneer Human Services is a nonprofit organization focused on supporting justice-involved individuals. It has operated for over 60 years and has more than 35 locations across Washington state.
