About The Position

The Health Care Coordinator serves as a liaison between Operations and Medical Services to ensure healthcare-related tasks, appointments, documentation, medication administration records, and revenue-generating clinical documentation are completed accurately and timely across assigned markets. This position provides oversight, auditing, coordination, and accountability support to ensure individuals receive necessary healthcare services while maintaining regulatory compliance and organizational standards. The Health Care Coordinator will work closely with residential staff, market leadership, nursing personnel, and external healthcare providers to coordinate appointments, monitor follow-through, identify documentation gaps, and ensure completion of assigned healthcare-related responsibilities.

Requirements

  • High school diploma or GED required.
  • Must pass criminal background checks, central registry check (in applicable states) and child abuse registry (in applicable states).
  • Must complete and pass a drug test, in applicable states.
  • Must possess a valid Driver’s License and have reliable transportation.
  • Proficient in speaking, reading and writing the English language.
  • Approved by state, federal and government entities to work within BSLS programs.
  • Ability to use office equipment, including BSLS information technology systems.
  • Strong organizational and time management skills.
  • Ability to prioritize multiple tasks and meet deadlines.
  • Understanding of healthcare documentation requirements.
  • Ability to identify discrepancies and follow through on corrective actions.
  • Strong attention to detail and auditing skills.
  • Excellent verbal and written communication abilities.
  • Ability to maintain confidentiality and professionalism.
  • Proficiency with Microsoft Office products and healthcare software systems.
  • Ability to work independently while collaborating with multiple departments.

Nice To Haves

  • Healthcare setting background preferred.
  • Experience in healthcare coordination, medical office administration, human services, nursing support, or related field preferred.
  • Experience working with Electronic Health Records (EHR) and electronic Medication Administration Records (eMAR) preferred.

Responsibilities

  • Schedule and coordinate medical, dental, behavioral health, specialty, and other healthcare-related appointments for individuals served.
  • Ensure all appointments are accurately entered into appropriate calendars and tracking systems.
  • Communicate appointment details to residential homes, supervisors, and other necessary stakeholders.
  • Verify transportation arrangements and ensure staff are aware of appointment requirements.
  • Confirm individuals are prepared and available for scheduled appointments.
  • Monitor appointment attendance and follow up on missed or rescheduled appointments.
  • Conduct routine audits of electronic Medication Administration Records (eMAR).
  • Identify and report uncharted medications and missed documentation.
  • Monitor medication availability and identify medications that are out of stock, unavailable, or nearing depletion.
  • Escalate medication concerns to appropriate clinical and operational leaders.
  • Track corrective actions and ensure identified issues are resolved in a timely manner.
  • Maintain audit records and provide reports to leadership.
  • Audit Electronic Health Records (EHR) to identify missing documentation, incomplete records, and data entry gaps.
  • Review records for documentation that may impact reimbursement, billing, funding opportunities, or service authorizations.
  • Partner with operations and clinical teams to ensure missing documentation is completed.
  • Track identified deficiencies and follow through until resolved.
  • Assist with maintaining accurate and compliant records.
  • Hold staff accountable for completion of assigned healthcare-related tasks and documentation requirements.
  • Monitor compliance with deadlines and organizational expectations.
  • Communicate unresolved concerns and barriers to appropriate leadership.
  • Provide reminders, tracking, and follow-up to ensure completion of required actions.
  • Support continuous improvement efforts related to healthcare operations and compliance.
  • Serve as a communication bridge between Operations and Medical Services.
  • Collaborate with nursing staff, market leaders, residential teams, and external providers.
  • Participate in meetings as requested.
  • Develop and maintain effective working relationships across departments.
  • Provide status updates and reports regarding audits, appointment coordination, and outstanding action items.
  • Assist with healthcare-related projects and initiatives.
  • Assist with staff scheduling needs
  • Support regulatory readiness and audit preparation activities.
  • Maintain confidentiality of protected health information (PHI).
