The Department of Health and Human Services is seeking to hire a passionate individual to fill their role of Health Care Coordinator. This role involves assessing and evaluating a patient's clinical problems and health care needs, providing care oriented toward improving or maintaining health and preventing illness, and providing clients with information concerning rights, options, benefits, services, goals, and expectations. The Health Care Coordinator will also maintain and update patient charts, records, and information, provide consultation and/or expert advice or testimony, and collaborate with various stakeholders, such as DCFS, Medicaid, and medical professionals, to ensure the client's healthcare needs are met. The Fostering Health Children Program's (FHCP) mission is to facilitate and advocate for excellence in medical, dental, mental health, and overall wellbeing for children in Utah’s foster care system. This program is designed to assist the Division of Child and Family Services (DCFS) in meeting the healthcare needs of Utah's children that are placed in foster care.
