About The Position

OCES is expanding its service area to 11 additional towns in the South Shore area. OCES supports older adults and individuals with disabilities by providing information and coordinating services to promote healthy, safe living and help consumers remain active and engaged in their communities. As part of the Community/Nutrition Program, the HC Meals Care Manager evaluates consumers’ need for home-delivered meal services and periodically reviews and reassesses their service needs. The role includes home visits, service planning and coordination, advocacy, documentation, communication with community resources, and visits to meal sites.

Requirements

• Bachelor’s degree in Social Work or a related field.
• Excellent verbal and written communication skills.
• Excellent interpersonal and conflict resolution skills.
• Excellent organizational skills and attention to detail.
• Strong analytical and problem-solving skills.
• Proficiency with Microsoft Office Suite and other software.

Nice To Haves

• Current social work license preferred.

Responsibilities

• Make home visits to assess consumers’ needs.
• Develop appropriate service plans and coordinate and monitor services rendered.
• Advocate for consumers in obtaining benefits and services, and assist as appropriate.
• Maintain documentation.
• Initiate and maintain communication with appropriate community resources to better coordinate delivery of consumer services as needed.
• Conduct visits to meal sites and collaborate with meal site and other OCES staff.
