About The Position

The Field Liaison coordinates services and identifies patients within the health system who may qualify for post-hospitalization services. The role assists with patient identification, hospital referrals, and prior authorization for services in accordance with facility standards.

The Field Liaison develops relationships and serves as an educational resource for referring providers, patients, and family members. The role coordinates care in a timely and efficient manner, identifies and removes barriers to care, and assesses patients’ physical, psychological, and social needs to enhance outcomes and satisfaction while reducing the cost of care. The Field Liaison is also responsible for community education and screening, monitoring community outreach, and promoting survivorship.

This PRN position is with the Provider Relations department at Holy Family Medical Center in Des Plaines, Illinois, with day-shift weekend coverage. Respiratory Therapist, Physical Therapist, or RN backgrounds are preferred.

Requirements

• Current BLS certificate upon hire and maintained current.
• Healthcare experience; a minimum of five years is preferred.

Nice To Haves

• Current and valid state LVN, PT, OT, RRT, or RN license preferred.
• Bachelor’s degree in healthcare or a closely related field preferred.
• Respiratory Therapist, Physical Therapist, or RN background preferred.
• Minimum of five years of healthcare experience preferred.

Responsibilities
