Scope of Position: The Residential Scattered Site Field Coordinator manages the day-to-day operations of the scattered site housing program, overseeing the coordination of tenant services and property management activities. This role involves facilitating move-ins and move-outs, scheduling and supervising repairs and maintenance, and ensuring compliance with housing regulations. The Field Coordinator serves as a liaison between tenants, landlords, and service providers, responds to emergency situations, and maintains accurate documentation and reports. The position also collaborates with various departments to enhance program effectiveness and address issues related to apartment readiness and tenant support.

Requirements

  • Bachelor’s degree required.
  • Driver’s license required.
  • Experience in facilities or property management.
  • Excellent time management and problem-solving skills.
  • Ability to adapt to change.
  • Stress-resistant, flexible, and a team player who is open to feedback.
  • Strong interpersonal, written, and verbal communication skills.
  • Experience working with individuals experiencing mental illness, providing crisis intervention, and managing trauma-triggered behaviors.
  • Willingness to obtain and maintain CPR, First Aid, AED, CPI, and Naloxone certifications.

Nice To Haves

  • Spanish-speaking ability is a plus.
  • Preferred knowledge of AWARDS.

Responsibilities

  • Provide restorative services and assistance with Activities of Daily Living (ADLs) to clients, including support with cooking, cleaning, apartment maintenance, shopping, transportation, medication management, symptom management, personal hygiene, money management, self-advocacy, and accessing community resources.
  • Escort clients to appointments and other essential community outings using public transportation for medical appointments, benefits access, shopping, and other necessary activities.
  • Motivate and support clients in developing skills to maintain housing while encouraging the development of natural supports and incorporating them into Support Plans and client meetings.
  • Ensure timely completion of required documentation for each client interaction in accordance with agency policies and regulatory guidelines for the applicable care level.
  • Co-facilitate groups on topics such as wellness, leadership, housing stability, employment, education, relationships, and decision-making, as needed.
  • Assist with preparation for program audits.
  • Provide crisis intervention when necessary.
  • Ensure compliance with program requirements, including proper admissions and discharges, maintaining accurate census records, completing documentation, and submitting incident reports.
  • Work with consumers, families, and staff in a respectful and culturally sensitive manner.
  • Conduct apartment inspections across multiple floors, utilizing stairs as needed.
  • Report serious incidents or concerns immediately to supervisors, complete required incident reports in accordance with Bridge policy, and take appropriate action if a client is unaccounted for.
  • Provide a supportive environment where clients can access services at any time of day or night, ensuring staff remain alert and available during all awake shifts.
  • Attend staff meetings, supervision sessions, and training sessions as required.
  • Perform other duties as assigned.
