About The Position

The Bridge offers supportive housing and behavioral health services to vulnerable New Yorkers facing behavioral health and substance use challenges. We provide individuals with the tools they need to pursue their dreams and live meaningful lives. Requirements

  • Minimum of a high school diploma or equivalent (GED).
  • Experience working within a harm reduction model or with high-risk populations.
  • Computer literacy required, including the ability to use the agency's electronic health record (EHR) system to document services.

Nice To Haves

  • Bilingual (Spanish) preferred.

Responsibilities

  • Provide intensive in-home harm reduction services and/or referral linkages for a small caseload of residents in Bridge housing throughout the Bronx, Manhattan, and Brooklyn.
  • Collaborate with internal departments and external providers to coordinate care.
  • Provide harm reduction guidance and training to supportive housing staff.
  • Promote clients' physical and mental health, safety, well-being, and recovery.
