About The Position
The Field CNA is a paraprofessional member of the interdisciplinary group who works under the supervision of a registered nurse. The CNA performs services as needed to meet patients’ personal needs and promote comfort, observes patients, reports observations, and documents observations and care provided. The position provides field care primarily in DuPage County, including Addison, Elmhurst, Lombard, and Oakbrook Terrace, with surrounding areas included based on census. The schedule is Monday through Friday, 8:00 AM to 4:30 PM. The role requires outside travel.
Requirements
- High school diploma or GED.
- Meet the training requirements of Illinois.
- Current CNA certification.
- Valid driver’s license.
- Dependable, properly insured vehicle that complies with state and organizational standards.
Responsibilities
- Provide compassionate care, emotional support, and companionship to patients and families.
- Assist patients with personal care, mobility, and activities of daily living.
- Monitor and document patient condition and report changes to the registered nurse.
- Ensure patient safety and proper use of equipment.
- Maintain a clean, safe, and comfortable home environment.
- Follow care plans and policies, and participate in training and team activities.
