Requirements

• Licensed Family Nurse Practitioner (FNP) or Physician Associate / Physician Assistant (PA)
• Current Illinois licensure
• Excellent clinical judgment, communication, and interpersonal skills
• A genuine passion for patient care and teamwork

Nice To Haves

• Two or more years of primary care experience preferred

Responsibilities

• Evaluate, diagnose, and treat patients across the lifespan
• Develop individualized treatment plans
• Prescribe medications and manage ongoing care
• Collaborate with physicians, specialists, therapists, and other healthcare providers
• Coordinate referrals and continuity of care
• Build and manage a thriving patient panel with the support of an experienced clinical and administrative team
• Maintain timely, accurate documentation within the electronic medical record
• Utilize telemedicine when appropriate
