Family Nurse Practitioner (FNP)
About The Position
The Center for Human Development (CHD) is seeking a Family Nurse Practitioner (FNP) for a brand-new role in West Springfield, MA. This position is designed around provider sustainability and clinical quality, allowing the FNP to help shape the structure of the role. The FNP will build patient relationships from day one with no inherited patient panel or backlogs. The schedule is Monday-Friday only, with no evenings, weekends, or routine on-call duties. Protected time for administrative and charting tasks is built into the workday. The role involves community-based care within the Easthampton, MA community, providing comprehensive primary care across the lifespan, including chronic disease management and preventative education. The FNP will have autonomy to practice evidence-based medicine within a supportive, multidisciplinary regional care team, with dedicated MA/RN support, embedded behavioral health colleagues, and visible medical leadership.
Requirements
- Master’s or Doctoral degree with FNP Certification.
- Valid MA APRN license.
- Strong judgment.
- Passion for community-based, patient-centered care.
