About The Position

We are seeking a Nurse Practitioner or Physician Assistant to join our collaborative Family Medicine team in Plainfield, CT. In this role, you will manage your own patient panel and work alongside 2 physicians and 2 APPs. You will have a dedicated MA per provider and a nursing team of three. The position offers the benefit of two nearby offices for easy coverage and work-life balance. This role is conveniently located near Providence, RI, and the scenic CT/RI shoreline, offering both professional fulfillment and access to beautiful weekend escapes.

Requirements

• Unrestricted State of Connecticut Medical License (or eligible)
• APRN (Family, Adult/Geri Nurse Practitioner) or PA training
• 2+ years’ post-training experience in a primary care or relevant outpatient setting

Responsibilities

• Perform routine physical examinations.
• Formulate medical, educational, and psychosocial plans to treat patients.
• Develop plans for further evaluation of health needs in consultation with physician(s) and care team members.
• Provide appropriate treatment for problems within the scope of the provider’s practice as designated by protocols.
• Identify problems outside of their scope of practice and refer appropriately to physician.
