About The Position
CenterWell Senior Primary Care is seeking a full-time Nurse Practitioner (NP) or Physician Assistant (PA) to provide senior-focused primary care. The role is based at the West Orange clinic in Orlando, Florida, with flexible coverage across the Orlando market. The position reports to the Associate Medical Director and works within a collaborative, multidisciplinary team that includes physicians, pharmacists, care coaches, behavioral health specialists, and other professionals. The clinician will provide comprehensive care to seniors in accordance with clinical standards and partner with the care team to coordinate personalized, high-quality services. Requirements
- Master's degree in Nursing or completion of a Physician Assistant program with board certification.
- Active, unrestricted Nurse Practitioner or Physician Assistant license in Florida.
- At least one year of experience in value-based care, primary care, or Medicare Advantage settings.
- At least one year of experience as an advanced practice provider.
Nice To Haves
- Active DEA license and Medicare/Medicaid provider numbers, or the ability to obtain a DEA license and/or Medicare and Medicaid numbers.
- Familiarity with HEDIS quality indicators and Medicare guidelines.
Responsibilities
- Provide comprehensive care to seniors in accordance with clinical standards.
- Collaborate daily with the care team to ensure coordinated, high-quality care.
- Manage referrals, hospital and skilled nursing facility coordination, durable medical equipment, and home health services.
- Document care accurately and efficiently, with support from quality-based coders.
- Participate in the on-call rotation.
- Support growth plans at the center.
