About The Position

Frontier Direct Care delivers direct primary care (DPC) to employer-sponsored patient populations. Providers manage a defined patient panel with an emphasis on continuity, preventive care, and chronic disease management, using lower daily patient volumes and longer visit times than traditional fee-for-service primary care. The Primary Care APP serves as the primary clinician, providing patient support, treatment, and management. This is a full-scope, hands-on primary care role involving chronic disease management, in-office procedures, and long-term patient relationships. Visits are conducted in person, by phone, and by secure text or video, and typically last 30 minutes to one hour depending on patient need. The clinician works with an assigned Medical Assistant and under a collaborating or delegating physician in accordance with Texas Medical Board supervision requirements. The role includes 24/7/365 on-call access for patient communication using a company-provided secure line.
