About The Position

KVC Kentucky is seeking a Family Care Coordinator to work directly with families on a day-to-day basis while managing and coordinating client care. The role focuses on in-home services that support child safety, permanency, and well-being, with the goal of reducing risk and preserving child placement with the family. The coordinator must maintain a flexible schedule, including nights and weekends as needed, travel throughout the region, and participate in an on-call rotation. A valid driver's license and proof of auto insurance are required.

Requirements

Bachelor’s degree in Social Work or another related field.
Ability to provide in-home services to children and families.
Experience working with complex and multi-need families.
Valid driver's license and proof of auto insurance.
Ability to maintain a flexible schedule, including nights and weekends as needed, travel throughout the region, and participate in an on-call rotation.

Responsibilities

Establish initial contact with families within 24 hours of referral to assess safety, initiate safety planning, and connect families with resources for immediate needs.
Conduct team meetings and maintain communication with families and providers to monitor treatment-goal progress and address barriers to services and goal attainment.
Facilitate implementation of each family’s Action Plan by contacting identified providers, connecting providers with the family, maintaining ongoing communication, and coordinating care throughout services.
Maintain frequent and consistent contact with DCBS personnel.
Provide direct services intended to reduce risks, improve child well-being, and preserve child placement with the family according to program guidelines.
