About The Position

The Care Coordinator Social Worker (CC-MSW) at Lankenau Medical Center is responsible for exemplary cross-continuum care coordination, including psychosocial assessments and interventions. The role works with physicians, peers, community colleagues, and others to coordinate care across the health care continuum, identify opportunities to improve patient care and services, improve population health, and support collaborative practices that reflect Main Line Health System’s commitment to patient-centered care and community engagement.

The position offers tuition reimbursement of up to $6,000 per year, based on full-time or part-time status. Position-specific benefits include up to 200 hours of paid time off per year based on employment status, employee discounts, and free employee parking.

Requirements

• At least 5 years of recent clinical experience in a physician’s office, clinic, hospital, emergency services (EMS) setting, or equivalent experience.
• Master’s degree in Social Work from a Council on Social Work Education (CSWE)-accredited program.

Nice To Haves

• Leadership experience is a plus.
• Licensed Social Worker; strongly preferred.
