Per Diem Care Coordinator Social Worker

About The Position
The Care Coordinator Social Worker (CC-MSW) at Lankenau Medical Center is responsible for cross-continuum care coordination, including psychosocial assessments and interventions. The role works with physicians, peers, community colleagues, and others to coordinate care across the health care continuum, identify opportunities to improve patient care and services, improve population health, and support collaborative practices aligned with Main Line Health System’s commitment to patient-centered care and community engagement. The position is per diem, with varied shifts mainly on weekends and some weekday coverage. The schedule is 16 hours per month, including one weekend per month and one holiday per year.
Requirements
- At least 5 years of recent clinical experience in a physician’s office, clinic, hospital, emergency services (EMS) setting, or equivalent experience.
- Master’s degree in Social Work from a CSWE-accredited program.
Nice To Haves
- Leadership experience.
- Licensed Social Worker licensure is strongly preferred.
