About The Position

St. Francis Hospital, The Heart Center® is seeking a per diem Licensed Master Social Worker (LMSW). The LMSW provides comprehensive social work services to patients and families, helping them navigate emotional, psychosocial, financial, and discharge-related needs. The role collaborates with the interdisciplinary care team to assess patient needs, provide support and counseling, and connect patients and families with community resources.

Requirements

• Master’s Degree in Social Work (MSW) from an accredited school of social work.
• Current New York State Licensed Master Social Worker (LMSW) license.
• 1–3 years of relevant social work experience.
• Experience with discharge and transitional planning.
• Knowledge of community resources, entitlement programs, and eligibility requirements.
• Strong counseling, crisis intervention, and de-escalation skills.
• Experience providing psychological and emotional support to patients and families.
• Knowledge and experience identifying, assessing, and reporting suspected abuse cases.
• SBIRT certification, or must obtain it as required by the organization.
• NYS DOH Screen Certification, or must obtain it as required by the organization.

Nice To Haves

• Healthcare-setting experience preferred.
• Experience in specialty areas such as dialysis, rehabilitation, substance use/ETOH, home care, oncology, or hospice preferred.

Responsibilities

• Provide psychosocial assessments, counseling, and emotional support to patients and families.
• Assist patients and families with discharge and transitional planning, including coordination of appropriate post-acute services and community resources.
• Educate patients and families about financial assistance programs, benefits, entitlements, and community resources.
• Identify and address psychosocial barriers that may affect treatment, recovery, or discharge plans.
• Provide crisis intervention and use appropriate de-escalation techniques when needed.
