About The Position

Sycamores provides a spectrum of mental health services to populations with varying levels of acuity, with a primary focus on low-income, high-risk youth. The Facilitator coordinates and organizes formal and informal supports in partnership with families to develop and implement a plan through a facilitated planning process and child and family team. The role helps create a unified plan across residential and community settings, supports timely transitions home, and follows children as they return to the community. The Facilitator collaborates with residential staff to maintain clear, open communication. Regular, flexible travel between client locations is an essential function of the position.

Requirements

• Bachelor’s degree.
• Two or more years of relevant experience.
• Maintain all required licenses and certifications.
• Personal, reliable vehicle for travel to various locations.
• Valid California driver’s license.
• Acceptable driving record as determined by Agency policy.
• Proof of auto insurance meeting minimum coverage requirements.
• Satisfy a pre-employment background check, drug screen, and health screen if presented with a conditional offer of employment.
