About The Position

We're looking for an outgoing and motivated External Client Care Coordinator who enjoys connecting with people, supporting families, and building lasting relationships throughout the community. If you're passionate about helping others while growing your career, we'd love to have you on our team. This role involves connecting with individuals and families to discuss their home care needs, guiding them through available services, and establishing/maintaining relationships with healthcare providers, community organizations, and referral partners. You will also visit clients to ensure outstanding care, partner with caregivers and office staff for service coordination, support new client onboarding, and participate in community outreach.

Requirements

  • Excellent relationship-building and communication skills
  • A friendly, compassionate, and customer-focused approach
  • Strong organizational skills with the ability to manage multiple responsibilities
  • Comfortable using computers and common office applications
  • Self-motivated with the ability to work independently and as part of a team
  • A positive attitude and a genuine desire to help others

Nice To Haves

  • Experience in healthcare, customer service, community outreach, client relations, or sales is an advantage but not required

Responsibilities

  • Connect with individuals and families to discuss their home care needs and guide them through available services
  • Establish and maintain relationships with healthcare providers, community organizations, and referral partners
  • Visit clients to ensure they are receiving outstanding care and address any questions or concerns
  • Partner with caregivers and office staff to coordinate services and provide a positive client experience
  • Support new client onboarding and help ensure a smooth transition into care
  • Participate in community outreach activities and represent Comfort Keepers at local networking events
  • Keep client information organized and maintain accurate records
  • Identify ways to improve client satisfaction and strengthen long-term relationships
  • Communicate regularly with families and the care team to ensure quality service
  • Represent Comfort Keepers with professionalism, compassion, and integrity
