Since 1983, our behavioral health therapists in Jacksonville, NC, have equipped patients with the skills they need to live happier, healthier, and more productive lives. We offer evidence-based treatment programs that address a number of issues, including grief, trauma, psychiatric disorders, and substance abuse problems. This position works in partnership with the clinician to serve the patient, and patient’s family through the coordination of discharge planning. Discharge planning includes coordinating transportation, setting up aftercare appointments, and faxing or scanning the discharge information to the aftercare provider. This person must possess discretion, the ability to act independently, good clinical judgment, creative thinking and problem-solving initiative. Strong communication skills, and the ability to exhibit empathy are a must. The person in this position must be able to demonstrate strong organizational skills and strong time management skills. Requirements

Bachelor’s degree in social work, counseling or a behavioral health related field.
Two years’ experience in a social services setting in a behavioral healthcare environment.
BLS/CPI certification.
Ability to exert 20 – 50 pounds of force occasionally or 10 – 25 pounds of force frequently or an amount greater than negligible and up to 10 pounds of force frequently or an amount greater than negligible and up to 10 pounds constantly to move objects.
Requires walking or standing to a significant degree.
Grasping; Speaking; Hearing; Computer use; Visual acuity (color, depth perception, field of vision); Sitting; Standing; Walking; Pushing; Pulling; Stooping; Reaching; Twisting; Crouching; Agility to perform physical restraints / holds; Agility to administer CPR.
Knowledge of OSHA: Blood Borne Pathogens Standards, CFR 1910.1030.
Understanding of appropriate policies, procedures, and plans related to blood borne pathogen exposure.

Responsibilities

Coordinate discharge planning, including transportation and aftercare appointments.
Fax or scan discharge information to aftercare providers.
Partner with clinicians to serve patients and their families.
