About The Position

The ER Discharge Coordinator supports centralized ER and Urgent Care follow-up programs by scheduling patient follow-up care at the bedside when appropriate. The role emphasizes a bedside-initiated patient experience to increase contact rates, improve scheduling immediacy, and enhance the overall patient experience. The coordinator helps maintain a positive, patient-centered, quality-driven environment and promotes the organization’s services and strategy.

The position is scheduled Monday through Friday, 12:00 p.m. to 8:30 p.m.

Requirements

• One year of related experience is preferred.

Responsibilities

• Schedule patients for follow-up care at the bedside when appropriate.
• Communicate pleasantly and professionally with customers, patients, and stakeholders.
• Communicate appropriately and clearly with department leadership, coworkers, and customers or patients.
• Ensure performance and quality standards are met.
• Provide superior customer service to internal and external customers and patients.
• Support the organization’s vision and strategy and positively promote its services.
• Elevate issues to leadership as appropriate.
• Suggest ideas for positive changes to department policies and procedures.
