About The Position

The ER Discharge Coordinator is responsible for point-of-care (PoC) patient bedside scheduling as part of the company’s centralized ER and Urgent Care follow-up programs. The role centers on a bedside-initiated patient experience intended to increase contact rates, enable immediate scheduling, and improve the overall patient experience. The coordinator creates and maintains a positive customer experience in a patient-centered, quality-driven environment.

Requirements

• One year of related experience is preferred.

Responsibilities

• Communicate pleasantly and professionally with customers, patients, and stakeholders.
• Communicate appropriately and clearly with department leadership, coworkers, and customers or patients.
• Schedule patients for follow-up care at the bedside when appropriate.
• Ensure performance and quality standards are met.
• Provide superior customer service to internal and external customers and patients.
• Support the organization’s vision and strategy, promote its services, and elevate issues to leadership when appropriate.
• Suggest ideas for positive changes to department policies and procedures.
