About The Position

The Director of Performance Improvement provides leadership for the Home Care Division’s performance improvement, quality, regulatory compliance, infection prevention, managed care, medical review, and risk management programs. This position works with clinical and operational leadership to improve quality outcomes, regulatory compliance, patient safety, and financial performance.

Requirements

  • Current New York State Registered Nurse (RN) license.
  • Master’s degree in Nursing, Healthcare Administration, Business Administration, Public Health, or a related healthcare field.
  • Minimum five years of progressive management experience in a Medicare-certified home health agency.
  • Strong knowledge of Medicare Conditions of Participation, OASIS, HHVBP, infection prevention, regulatory requirements, audits, appeals, and home health reimbursement.
  • Strong leadership, analytical, communication, presentation, and meeting facilitation skills.

Responsibilities

  • Leads the Home Care Quality Assessment and Performance Improvement (QAPI) Program.
  • Oversees regulatory compliance, survey readiness, corrective action plans, and regulatory education.
  • Monitors OASIS outcomes, HHVBP performance, quality measures, patient outcomes, and satisfaction.
  • Oversees Medicare and commercial payer audits, medical reviews, appeals, and related documentation improvement.
  • Leads Infection Prevention and Control activities, including surveillance, education, reporting, and outbreak response.
  • Provides oversight of Managed Care compliance, authorization processes, and quality reporting.
  • Conducts performance analysis, root cause analyses, and improvement initiatives using clinical, operational, financial, and regulatory data.
  • Collaborates with Home Care and Catholic Health leadership to promote patient safety, quality improvement, and regulatory compliance.
  • Presents performance data, trends, and recommendations to leadership, committees, and governing bodies.
  • Leads assigned performance improvement meetings and committees.
