About The Position

Lorian Health is seeking an experienced Registered Nurse to lead its home health clinical team as Director of Patient Care Services. The role drives high-quality, patient-centered care, ensures regulatory compliance, and supports a collaborative clinical environment. The Director leads home health clinical operations and staff, oversees care delivery, and coordinates with physicians and interdisciplinary care teams.

Requirements

• Active RN license.
• At least 3 years of experience in home health or healthcare.
• Strong knowledge of Medicare and Medi-Cal regulations.
• Excellent leadership, communication, and organizational skills.
• Experience with EMR systems and Microsoft Office.

Responsibilities

• Lead and supervise all home health clinical operations and staff.
• Ensure quality patient care in accordance with physician orders and care plans.
• Maintain compliance with Medicare, Medi-Cal, and state regulations.
• Oversee staff hiring, training, performance, and scheduling.
• Drive performance improvement and ensure accurate clinical documentation.
• Coordinate interdisciplinary care and serve as a key liaison with physicians.
• Manage patient admissions, discharges, and overall care delivery.
