About The Position

A healthcare facility near Portland, Oregon, is seeking a Director of Patient Care Services to lead its home health team. This is a full-time, permanent, direct-hire position in a home health setting, with a Monday through Friday schedule from 8 AM to 5 PM. Compensation starts at $120,000, depending on experience. Requirements

• Active and unencumbered RN license.
• At least 4 years of recent home health experience.
• Previous management-level experience in home health, ideally at the manager, supervisor, or director level.

Responsibilities

• Maintain adherence to state regulations, federal laws, and Medicare Conditions of Participation.
• Direct patient care plans, admissions, recertifications, and multidisciplinary case conferences.
• Lead a team of clinical staff and administrative support personnel.
• Provide leadership, guidance, and mentorship to promote high-quality patient outcomes and strong team collaboration.
• Oversee documentation accuracy, survey readiness, and interdisciplinary team communication.
