About The Position
A healthcare facility near Portland, Oregon, is seeking a Director of Patient Care Services to lead its home health team. This is a full-time, permanent, direct-hire position in a home health setting, with a Monday–Friday schedule from 8 AM to 5 PM. Compensation starts at $120,000 and is based on experience. The role includes leading clinical and administrative personnel and overseeing patient care, regulatory compliance, documentation, survey readiness, and interdisciplinary communication.
Requirements
- Active, unencumbered RN license.
- At least 4 years of recent home health experience.
- Previous management-level experience in home health, ideally at the manager, supervisor, or director level.
Responsibilities
- Maintain adherence to state regulations, federal laws, and Medicare Conditions of Participation.
- Direct patient care plans, admissions, re-certifications, and multidisciplinary case conferences.
- Lead clinical staff and administrative support personnel, providing leadership, guidance, and mentorship to support high-quality patient outcomes and strong team collaboration.
- Oversee documentation accuracy, survey readiness, and interdisciplinary team communication.
